Ventrilo can be a little daunting to setup for the first time user, so below is a screenshot of the first tab in the setup dialogue box.
Please note that certain settings (i.e., input device) will not be the same: My sound card is an "SB Audigy 2ZS Audio [D000]", whereas your choice(s) may be something like "Default DirectSound device" or "Leadtek SD" etc.

These devices are used to define your audio hardware / software that you need to select in the "Output Device" and "Input Device" drop-down list boxes, please use the screenshot as a guide. Where "Default DirectSound device" is selected (for output device), please use this (providing you have that option), and make sure that all the relevant check-boxes are ticked.
In relation to the "Hardware Input Mixer (Optional)" settings, you may be successful in getting options to appear in the drop-down list boxes, but if not, don't worry - it is optional after all. Sometimes you will get an error message about "Missing Mux for mixer". Once this has appeared and you have closed it, it should not appear again.
One of the common issues with Ventrilo is that the user's voice either comes through too soft or too loud enough to create unintelligible distortion to the listeners at the other end.
This can be remedied via the "Amplifiers" section:
The "Outbound" Slider governs the volume of your transmission; slide it to the right it get's louder, to the left it gets softer.
The "Inbound Slider" governs the volume you hear and is operated in the same fashion to the Outbound slider.
Another setting that you may need to check is via the "Recording (Input)" button. When clicked you should see a "Recording Control" window with slider controls for "Midi Synth", "Analog Mix", "Microphone" and "Wave". Please make sure that the "Microphone" Slider is all the way to the top and that the "Select" Checkbox is ticked, then close the window.
In relation to the "Hotkey", please click in the box next to it and then choose your key, many users use their left "Alt" key because it is conveniently close during gaming, however, this is a personal choice.
Once you have all the settings completed on the "Voice" tab, (pictured), you can elect to test your mike and headphones / speakers using the "Test With" features (lower left).
Finally, join us in ventrilo and have a great time.
For those unfortunate few who may still have problems, please still sign into our ventrilo channel, click on the "Comment" button and type in a quick message like "Help needed with Vent" in the top text box. Someone (probably myself) will notice you in due course and come to your aid.
If all else fails, please PM me with the details of your soundcard, microphone and headset / speaker hardware, and I will check around to see if their are any known issues and work arounds to be found.
